Modalidades de diagnóstico por imágenes en la retinopatía del prematuro: revisión crítica de la evidencia reciente

Resumen
La retinopatía del prematuro (ROP) es una enfermedad vasoproliferativa que afecta a neonatos prematuros y constituye una de las principales causas prevenibles de ceguera infantil en todo el mundo. Su diagnóstico estándar, la oftalmoscopia indirecta, requiere especialistas, resulta estresante para el neonato y presenta considerable variabilidad entre observadores. En la última década, se ha explorado el valor de la ecografía modo B, el Doppler de alta frecuencia, la resonancia magnética (RM) y la tomografía computarizada (TC) como herramientas complementarias. Una revisión de la literatura identificó estudios clave que muestran que la ecografía B scan detecta con fiabilidad desprendimientos traccionales “en embudo” y complicaciones asociadas en etapas avanzadas de la enfermedad; el Doppler ultrarrápido ha revelado flujos retinianos significativamente mayores en ROP severa, ofreciendo un enfoque cuantitativo para estratificar el riesgo; la RM orbitaria diferencia un desprendimiento de retina sin masa tumoral de un retinoblastoma y la RM cerebral ha asociado la ROP a menor volumen de sustancia blanca y cerebelo y a peores resultados neurocognitivos a los dos años; por último, la TC se reserva para descartar retinoblastoma mediante detección de calcificaciones intraoculares. Aunque estas modalidades no reemplazan el cribado oftalmológico en etapas tempranas, aportan información anatómica y hemodinámica valiosa en casos avanzados y en diagnósticos diferenciales, allanando el camino hacia un manejo más objetivo y multidisciplinario de la ROP.

Abstract
Retinopathy of prematurity (ROP) is a vasoproliferative disease affecting preterm infants and remains one of the leading preventable causes of childhood blindness worldwide. The standard diagnostic method, indirect ophthalmoscopy, requires specialized expertise, can be stressful for the infant, and shows significant interobserver variability. Over the past decade, ocular B scan ultrasound, high frequency Doppler, magnetic resonance imaging (MRI), and computed tomography (CT) have been investigated as complementary tools. Key studies demonstrate that B scan ultrasound reliably identifies tractional “funnel shaped” retinal detachments and associated complications in advanced ROP; ultrafast Doppler ultrasound reveals markedly increased retinal blood flow in severe cases, offering objective risk stratification; orbital MRI differentiates total retinal detachment without an intraocular mass from retinoblastoma, and cerebral MRI has linked ROP to reduced white matter and cerebellar volumes and poorer neurodevelopmental outcomes at two years; finally, CT is limited to ruling out retinoblastoma by detecting intraocular calcifications. While these imaging modalities do not replace routine ophthalmic screening in early disease, they provide valuable anatomical and hemodynamic information in advanced stages and differential diagnosis, paving the way for a more objective, multidisciplinary approach to ROP management.
