Optimización preoperatoria integral en estenosis aórtica severa del adulto mayor: convergencia estratégica de la medicina interna, cardiología, cirugía cardíaca y anestesiología para maximizar los resultados

Resumen

La estenosis aórtica degenerativa afecta a > 4 % de los octogenarios y a 12 % de los ≥85 años (1); cuando es grave y sintomática, la supervivencia sin intervención se limita a ~2 años (2). El reemplazo valvular aórtico quirúrgico o transcatéter mejora la sobrevida, pero la multimorbilidad y la fragilidad elevan el riesgo perioperatorio (3). Se revisó la literatura 2015 2025 (PubMed, Scopus, Embase, SciELO, Google Académico) con términos MeSH/DeCS; se analizaron críticamente 20 fuentes de alto nivel. La valoración geriátrica integral descubre en promedio seis problemas no diagnosticados por paciente—malnutrición, polifarmacia, fragilidad, deterioro cognitivo—y permite corregirlos antes de la cirugía (5). La discusión colegiada en Heart Team optimiza la elección entre TAVI y cirugía y reduce la mortalidad intrahospitalaria y los eventos adversos inmediatos (4). La optimización agresiva de comorbilidades, los programas breves de prehabilitación físico nutricional y la planificación anestésica individualizada se asocian a menos fibrilación auricular posoperatoria, menor estancia hospitalaria y mejor recuperación funcional (8). La evidencia respalda que combinar evaluación geriátrica, control de comorbilidades, prehabilitación, soporte anestésico especializado y decisiones en Heart Team disminuye la morbimortalidad y acelera la recuperación tras el reemplazo valvular en adultos mayores. La detección sistemática de la fragilidad y la inclusión rutinaria del geriatra o internista en los protocolos prequirúrgicos son esenciales para maximizar el beneficio clínico y la eficiencia asistencial.

Abstract

Degenerative aortic stenosis affects > 4 % of octogenarians and 12 % of individuals aged ≥ 85 years (1); when severe and symptomatic, median survival without intervention is ~2 years (2). Surgical or transcatheter aortic valve replacement improves survival, yet multimorbidity and frailty markedly heighten peri operative risk (3). A 2015 2025 search of PubMed, Scopus, Embase, SciELO and Google Scholar using MeSH/DeCS terms retrieved meta analyses, RCTs, guidelines and cohort studies; twenty high quality sources were critically analysed. Comprehensive geriatric assessment uncovers a mean of six previously unrecognised problems per patient—malnutrition, polypharmacy, frailty, cognitive decline—allowing targeted correction before surgery (5). Heart Team discussion refines selection between TAVR and surgery and lowers in hospital mortality and early adverse events (4). Aggressive optimisation of comorbidities, short physical nutritional prehabilitation programmes and tailored anaesthetic planning are linked to less postoperative atrial fibrillation, shorter hospital stays and better functional recovery (8). Evidence supports that combining geriatric assessment, comorbidity control, prehabilitation, specialised anaesthesia and Heart Team decision making reduces peri operative morbidity and mortality and accelerates recovery after valve replacement in older adults. Systematic frailty screening and routine involvement of a geriatrician or internist in pre surgical protocols are essential to maximise clinical benefit and healthcare efficiency.
