Abstract

Psychosis was reinterpreted as a disturbance of distributed networks rather than as a phenomenon explained only by neurotransmitters. This scoping review mapped evidence on invasive and noninvasive neuromodulation directed at neural circuits in psychotic disorders. Reproducible searches were designed for PubMed/MEDLINE, Scopus, Embase, ScienceDirect/Elsevier and UpToDate, the latter only as secondary clinical context. Evidence was most consistent for repetitive transcranial magnetic stimulation in auditory hallucinations and negative symptoms, and more preliminary for electrical stimulation, deep brain stimulation and focused ultrasound. Methodological heterogeneity, small samples and limited connectivity-guided personalization persisted. Circuit-based neuromodulation emerged as a promising field, but still complementary to standard treatment.
