Abstract
Introduction. Anterior cruciate ligament (ACL) injuries affect knee stability and strength, lead to a longer recovery time before returning to soccer, and can cause apprehension regarding future injury. These injuries, combined with the effects of hydrostatic pressure, buoyancy, temperature, and water resistance, are the reasons why aquatic therapy is used during rehabilitation. Objective. This study analyzes the effects of aquatic therapy on pain, mobility, strength, function, and return to sport, as well as participant safety, in individuals with ACL injuries or reconstructions, focusing on soccer players. Methods. A systematic review was conducted in accordance with PRISMA 2020 guidelines. Spanish-language publications were searched in the PubMed/MEDLINE, SciELO, Dialnet, SPORTDiscus, PEDro, and Cochrane Library databases, and reference lists were screened, covering the period from January 2021 to August 2026. A narrative synthesis of 11 articles was performed, with three articles focusing directly on aquatic therapy. Risk of bias was assessed using ROBINS-I, RoB 2, or PEDro tools, and the certainty of evidence was evaluated using the GRADE approach. Results. Studies on aquatic therapy have shown pain reduction and improvements in mobility and function, whereas studies comparing land-based and aquatic therapies have yielded mixed results regarding quadriceps muscle strength improvement. No recent studies have focused on the return to soccer match play or injury recurrence. Conclusion. Current evidence supports the use of aquatic therapy during the initial phases of rehabilitation and indicates its utility for performing therapy with reduced joint impact.
