Síndrome de Wallenberg en mujer de 50 años: Informe clínico y revisión del enfoque diagnóstico

Resumen

Introducción: El síndrome de Wallenberg (infarto bulbar lateral) es una forma clásica de ictus del territorio vertebrobasilar, con manifestaciones cruzadas sensitivas, signos vestíbulo-cerebelosos y compromiso bulbar. Su reconocimiento temprano es clave porque puede simular trastornos periféricos y porque la tomografía simple suele ser poco sensible para lesiones del tronco encefálico. Caso clínico: Presentamos a una mujer de 50 años con inicio súbito de vértigo intenso, vómitos, disfonía y disfagia, asociadas a ataxia y signos autonómicos (síndrome de Horner). La evaluación oculomotora tipo HINTS fue sugestiva de causa central. La resonancia magnética con difusión evidenció un infarto dorsolateral bulbar izquierdo, y la angiografía por tomografía mostró hallazgos compatibles con disección de la arteria vertebral. Conclusión: El enfoque diagnóstico debe integrar semiología de tronco encefálico, exploración oculomotora en el síndrome vestibular agudo y neuroimagen dirigida (RM-DWI y angio-TC/angio-RM). Una RM inicial negativa no excluye un evento vertebrobasilar, por lo que puede requerirse repetición ante alta sospecha clínica.

Abstract

Background: Wallenberg syndrome (lateral medullary infarction) is a posterior circulation stroke syndrome that may mimic peripheral vestibular disorders, and non-contrast CT can miss early brainstem lesions. Case: We report a 50-year-old woman with sudden-onset vertigo, vomiting, dysphonia, and dysphagia with ipsilateral Horner syndrome and ataxia. A HINTS examination suggested a central cause. Diffusion-weighted MRI confirmed a left dorsolateral medullary infarction and CT angiography supported vertebral artery dissection. Conclusion: Diagnosis relies on careful brainstem semiology plus targeted imaging (MRI-DWI and vascular imaging). Early DWI can be false-negative in posterior circulation events; repeat imaging should be considered when clinical suspicion remains high.
