Conocimiento y prácticas de autocuidado en adultos con diagnóstico de diabetes tipo 2

Resumen

Objetivo: Determinar la relación entre el nivel de conocimiento y las prácticas de autocuidado en adultos con diagnóstico de diabetes mellitus tipo 2 atendidos en establecimientos de salud de primer nivel de la ciudad de Machala. Metodología: Estudio cuantitativo, no experimental, descriptivo-correlacional y de corte transversal, desarrollado en 124 adultos con diabetes mellitus tipo 2. Para la recolección de datos se aplicaron dos instrumentos: el Diabetes Knowledge Questionnaire-24, para valorar el nivel de conocimiento, y el Summary of Diabetes Self-Care Activities, para evaluar las prácticas de autocuidado. El análisis estadístico incluyó frecuencias, porcentajes, medias, desviación estándar y prueba t de Student, con significancia estadística de p < 0,05. Resultados: El 59,7% de los participantes presentó conocimiento suficiente sobre diabetes mellitus tipo 2, mientras que el 40,3% evidenció conocimiento no suficiente. En las prácticas de autocuidado, la mayor adherencia se observó en el cuidado de los pies, dieta y actividad física; en contraste, el automonitoreo de glucosa mostró menor cumplimiento. Al comparar las prácticas según nivel de conocimiento, se encontraron diferencias significativas en actividad física, con mayor frecuencia en quienes tenían conocimiento suficiente, y en prueba de azúcar en sangre, con mayor frecuencia en quienes presentaron conocimiento no suficiente. Conclusión: Aunque la mayoría de los adultos evaluados alcanzó un nivel suficiente de conocimiento, este no se relacionó de forma uniforme con mejores prácticas globales de autocuidado. Se evidencia la necesidad de fortalecer intervenciones educativas de enfermería orientadas no solo a incrementar conocimientos, sino también a promover conductas sostenidas de autocuidado.

Abstract

Objective: To determine the relationship between the level of knowledge and self-care practices among adults diagnosed with type 2 diabetes mellitus attending primary health care facilities in the city of Machala. Methodology: A quantitative, non-experimental, descriptive-correlational, cross-sectional study was conducted with 124 adults diagnosed with type 2 diabetes mellitus. Data were collected using two instruments: the Diabetes Knowledge Questionnaire-24, to assess the level of knowledge, and the Summary of Diabetes Self-Care Activities, to evaluate self-care practices. Statistical analysis included frequencies, percentages, means, standard deviations, and Student’s t-test, with statistical significance set at p < 0.05. Results: A total of 59.7% of participants had sufficient knowledge about type 2 diabetes mellitus, while 40.3% showed insufficient knowledge. Regarding self-care practices, the highest adherence was observed in foot care, diet, and physical activity; in contrast, blood glucose self-monitoring showed the lowest adherence. When self-care practices were compared according to level of knowledge, significant differences were found in physical activity, with higher frequency among participants with sufficient knowledge, and in blood glucose testing, with higher frequency among those with insufficient knowledge. Conclusion: Although most adults assessed reached a sufficient level of knowledge, this was not uniformly associated with better overall self-care practices. These findings highlight the need to strengthen nurse-led educational interventions aimed not only at increasing knowledge, but also at promoting sustained self-care behaviors.
