INTESTINAL OBSTRUCTION IN A 5-YEAR-OLD MALE PATIENT: CASE REPORT

Abstract

Introduction: Intestinal obstruction in pediatrics is a surgical emergency with etiologies that vary by age. In preschool and school-aged children, in addition to ileocolic intussusception, less common causes such as congenital bands and complications of Meckel's diverticulum should be considered. Clinical case: A previously healthy 5-year-old boy presented with 24-hour complaints of cramping abdominal pain, bilious vomiting, distension, and absence of bowel movements and gas. X-rays showed air-fluid levels; ultrasound revealed thin, dilated loops with back-and-forth peristalsis. After resuscitation and nasogastric decompression, diagnostic laparoscopy revealed a mesodiverticular band causing obstruction of the distal ileum. The band was released, and a diverticulectomy was performed. The patient's outcome was favorable without complications, and the patient was discharged on the fourth day. Conclusion: In children without prior surgery, suspicion of non-adhesive causes of obstruction, such as congenital or Meckel bands, should be high. Ultrasonography guides the diagnosis, and laparoscopy offers confirmation and treatment in the same procedure.
