Hernia inguinal congénita complicada con incarceración y perforación intestinal en paciente masculino de 1 año: reporte de caso

Resumen

La hernia inguinal congénita es una de las causas más frecuentes de consulta quirúrgica en pediatría. Aunque la mayoría de los casos son reductibles y se manejan de manera electiva, la incarceración puede progresar a estrangulación, isquemia y, en situaciones excepcionales, perforación intestinal. Presentamos el caso de un paciente masculino de 1 año de edad que acudió por distensión abdominal, vómitos y fiebre, asociado a aumento de volumen inguinoescrotal derecho no reductible. La radiografía abdominal sugirió obstrucción intestinal y los exámenes complementarios mostraron anemia y respuesta inflamatoria. Se realizó laparotomía exploratoria de urgencia, evidenciando un asa intestinal comprometida en el saco herniario, con signos de hipoperfusión y perforación, además de líquido inflamatorio intraperitoneal. Se efectuó reparación intestinal y herniorrafia inguinal derecha. El paciente evolucionó favorablemente, con estancia breve en cuidados intensivos y alta hospitalaria con controles programados. Este caso subraya la necesidad de reconocer oportunamente los signos de hernia inguinal complicada y priorizar la resolución quirúrgica urgente para prevenir complicaciones potencialmente graves (1,2,3).

Abstract

Congenital inguinal hernia is one of the most frequent reasons for surgical consultation in pediatrics. Although most cases are reducible and managed electively, incarceration can progress to strangulation, ischemia, and, in exceptional situations, intestinal perforation. We present the case of a 1-year-old male patient who presented with abdominal distension, vomiting, and fever, associated with a non-reducible right inguinoscrotal swelling. Abdominal radiography suggested intestinal obstruction, and complementary tests showed anemia and an inflammatory response. An emergency exploratory laparotomy was performed, revealing a loop of intestine compromised within the hernial sac, with signs of hypoperfusion and perforation, as well as intraperitoneal inflammatory fluid. Intestinal repair and right inguinal herniorrhaphy were performed. The patient recovered favorably, with a short stay in intensive care and discharged with scheduled follow-up appointments. This case underscores the need to recognize the signs of complicated inguinal hernia early and prioritize urgent surgical intervention to prevent potentially serious complications (1,2,3).
