Nuevos esquemas de estimulación ovárica en infertilidad: revisión bibliográfica

Introducción. La estimulación ovárica leve, es decir, la de baja complejidad, en donde se obtienen no más de 4 óvulos, se propuso hace más de 20 años como un régimen alternativo a la estimulación ovárica convencional para mujeres que se someten a procedimientos de FIV, la cual se basa en la suposición de que las dosis bajas y/o la duración breve de la gonadotropina exógena pueden promover el reclutamiento de pocos folículos que contengan los ovocitos más competentes. Objetivo. Exponer los nuevos esquemas de estimulación ovárica en pacientes con infertilidad mediante revisión bibliográfica. Metodología. Revisión bibliográfica tipo narrativa en estudios, metaanálisis, revisiones sistemáticas, ensayos clínicos controlados, estudios de cohortes, revisiones bibliográficas que reporten información sobre los nuevos esquemas de estimulación ovárica para pacientes con infertilidad, desde 2018 a 2023, en los idiomas inglés y español. Resultados. se escogieron 17 artículos, donde encontramos que existen diferentes tipos de gonadotropinas en el mercado, urinarias y recombinantes, presentando ambas con igual tasa de eficacia, la decisión de la elección entre una u otra dependerá de los criterios individuales del clínico, que tendrá en cuenta la relación coste-beneficio. La Corifolitropina alfa es una nueva molécula hibrida que presenta la misma capacidad de desarrollo multifolicular en la estimulación ovárica que los preparados actuales en el mercado, con la ventaja de que tiene una vida media más larga. Conclusión: actualmente se dispone de muchos protocolos de estimulación ovárica. Lo importante es, además de conocerlos todos, saber cuál de ellos se adapta mejor a cada paciente.

Abstract
Introduction. Mild ovarian stimulation, that is, low complexity, where no more than 4 eggs are obtained, was proposed more than 20 years ago as an alternative regimen to conventional ovarian stimulation for women undergoing IVF procedures, which assumes that low doses and/or short duration of exogenous gonadotropin can promote the recruitment of few follicles containing the most competent oocytes. Objective. To present the new ovarian stimulation schemes in patients with infertility through a bibliographic review. Methodology. Narrative bibliographic review in studies, meta-analysis, systematic reviews, controlled clinical trials, cohort studies, bibliographic reviews that report information on new ovarian stimulation schemes for patients with infertility, from 2018 to 2023, in English languages and Spanish. Results. Seventeen articles were chosen, where we found that there are diverse types of gonadotropins on the market, urinary and recombinant, presenting both with the same rate of efficacy. The decision to choose between one or the other will depend on the individual criteria of the clinician, who will have considered the cost-benefit ratio. Corifolitropina alfa is a new hybrid molecule that has the same capacity for multifollicular development in ovarian stimulation as the current preparations on the market, with the advantage that it has a longer half-life. Conclusion. Currently there are many ovarian stimulation protocols available. The important thing is, in addition to knowing them all, to know which one best suit each patient.
